We work closely with social workers, care coordinators, GP practices, hospital discharge teams, and integrated care boards across Kent. What to Include in a Referral
- Service user's name, age, and current location
- Type of care or placement required
- Urgency and target start date
- Funding type (LA commissioned, CHC, CHC fast-track, private)
- Brief overview of current support needs and any complex requirements
- Your name, organisation, and direct contact details
- Any relevant reports or assessments (can be submitted by email)
